To register as an attending physician, please complete the fields provided.

• Please use your personal email address. Do not use a general office manager email address. Your assessment is tied to this email. Your CME certificate will be presented at the meeting.
• Proof of full vaccination and face masks required for in-person attendance.

If a cancellation is received in writing up to two weeks prior to the event, the participant will receive a refund, minus a $50 cancellation fee. There will be no refunds for cancellations received after that date.
